Started applying this cream 2 days ago, he said it could be thrush or nappy rash as she is really teething. the rash looks really red %26amp; sore now, much worse than wednesday and she is waking up screaming.

I think that cream is an anti-fungal and only really suitable if your baby has a fungal infection like thrush. It may well be the rash is from an allergy in which case that cream will not work so def go back to your GP to get a cream that works for other types of infections.

Definitely get your baby back into the pediatrician as soon as possible. Your baby is probably allergic to the creme.
He/She may be allergic to the cream consult your doctor that prescribed it and try going to the pedetrician
